CHRONIC ILLNESS

A chronic illness is an illness that has been certified, within the past 12 months, by a licensed health care practitioner, where an individual is unable to perform, without substantial assistance, at least two out of six activities of daily living - ADLs - for a period of at least 90 days due to a loss of functional capacity and will require continuous care for the remainder of life.

 The six activities are defined as:

  1.  eating,
  2. toileting,
  3. transferring,
  4. bathing,
  5. dressing and
  6. continence.
  7. Cognitive impairment may also qualify
  • Heat Disease

  • Lung Cancer

  • Osteoporosis

  • Colorectal Cancer

  • and others
